Estrella is still flat-lining. Yosh runs in and says that her CBC and lytes are normal. Mark mutters, "What the hell is wrong with this baby?" and then tells them to start "pacing at one-eighty." Long story short, the good thing that should happen, doesn't, and Mark finally calls it, and tells them to clean up the baby before he brings in her mother.

Kim tells Benton he doesn't want the surgery. Benton starts to say, "It's the only way --" but Kim cuts him off and says he understands, but that he doesn't to go through surgery again. Benton looks at Lisa, who resolutely avoids looking back at him. His pager goes off, and he tells Kim that they could treat him with antibiotics and hope for the best. Kim nods. Benton apologizes and excuses himself. Lisa goes after him and asks, "Aren't we supposed to fix our mistakes?" Benton says, "Usually," and adds that Kim has end-stage cancer, and that surgery would only prolong his suffering. Benton adds that Lisa has to respect Kim's wishes, and Lisa squeaks, "But he's going to die because I made a mistake!" Benton reminds her, "He's going to die anyway." Lisa frowns.

Chuny calls Mark into a trauma room. Mrs. E. starts talking rapidly in Spanish. Chuny quietly asks, "You want me to give her something?" Mark says no. Mrs. E. implores Mark to help Estrella. He wearily says, "Pilar," and Pilar (formerly Mrs. E.) says that Estrella's still warm, and that there must be something he can do. Mark tells her they tried everything, but Pilar isn't getting it, and begs him to use the machines, or blow in Estrella's mouth to revive her. Mark firmly says that they can't, and Pilar sobs, "Yes, you can! Yes, you can! You're a good doctor!" Flights of fancy aren't going to bring back your daughter, Pilar. Mark says, "Pilar. She's dead. I'm sorry." Pilar cries. Mark comforts her. Even I am not going to make jokes about a dead baby.
